COVID-19 VACCINE

  • We have the Moderna vaccine. Please call the office at 330-494-7099 or send a portal message to schedule an appointment. 

 

We are recommending that our patients be immunized.

COVID-19 OFFICE PRECAUTIONS

  • Everyone is required to wear a mask in our building and will be requiring masks after June 2nd.

  • Do not enter the building if you have an of following symptoms, cough, fever, chills, muscle pain, sore throat, shortness of breath, runny nose, loss of taste or smell, call the office. 

  • All sick visits will be done outside at the back of the building via a car side appointment.

  • If you are interested in changing your routine appointment to a telehealth visit, please call the office. 

  • If you have an appointment and would like to wait in your car, please check-in at the desk and let reception know.  We will call you when the provider is ready.
